2. COLLECTION OF PERSONAL & HEALTH INFORMATION
We collect personal and sensitive health information necessary to provide clinical telehealth services, including:
• Contact details (name, email address, phone number, residential address).
• Health information (medical history, GP details, Mental Health Treatment Plans, referral letters, and session clinical notes).
• Billing information (Medicare details, private health insurance information, payment processing details).
3. HOW INFORMATION IS COLLECTED AND STORED
• Website Inquiries: Information submitted via our website form is transmitted securely and stored in encrypted form.
• Telehealth Consultations: Telehealth sessions take place via end-to-end encrypted video platforms. Audio or video recordings of sessions are strictly prohibited without prior written consent.
• Practice Records: Clinical notes and personal data are stored securely in compliance with Australian digital health data security standards.
4. USE AND DISCLOSURE
Your information is kept confidential and will only be disclosed in the following circumstances:
• To deliver psychological assessment and therapy.
• For administrative and billing purposes (e.g., claiming Medicare rebates or processing private health claims).
• To communicate with your referring GP or treating medical practitioner, where appropriate.
• Where required or authorised by law (e.g., subpoena, court order, or mandatory reporting requirements).
• Where there is reasonable belief that failure to disclose information would place you or another individual at serious, imminent risk of harm.
5. ACCESS AND CORRECTION
You have the right to request access to the personal health information we hold about you, subject to exceptions permitted by law. You may also request corrections if you believe the information is inaccurate or outdated. Requests should be directed in writing to admin@spokenwordpsychology.com.au.
